Understanding Support at Home and Care Management in Aged Care

Care Management through a Care Partner in Support at Home

Under the Australian Government’s Support at Home program  people receiving ongoing services have access to care management.

Care management ensures your services are regularly reviewed, well-coordinated, and aligned with your needs and goals.

At Chris Barnard Health, your Care Partner delivers much of this care management in a practical, personal way. 

Care Management Includes:

Care planning

  • Understanding your needs, goals, and supports
  • Creating and reviewing your care plan
  • Updating your service agreement when needed

Service coordination

  • Communicating with care workers and health professionals
  • Coordinating family involvement (with consent)
  • Supporting hospital-to-home transitions
  • Managing service changes and budgets

Monitoring and review

  • Checking in on your wellbeing and progress
  • Identifying changes or risks early
  • Reviewing goals and service outcomes
  • Adjusting care when needed

Support and guidance

  • Helping you understand services and funding
  • Supporting decision-making
  • Providing information and resources
  • Assisting with feedback or concerns
  • Supporting wellbeing, independence, and reablement

What Does Care Management Cost?

If you receive ongoing Support at Home services, 10% of your budget each quarter is allocated to care management.

This is a government requirement and is taken from your existing funding – not an additional out-of-pocket cost.

It applies even if you choose to self-manage (not available at Chris Barnard Health) some or all of your services.
