Nursing Care at Home for Veterans and Their Families

DVA-funded nursing and clinically required personal care in your own home, supporting both physical and eligible mental health-related care needs.

DVA-Eligible? Care at No Cost

Eligible clients receive DVA Community Nursing at no cost to them. Let CBH help you understand the pathway and coordinate the care you need at home.

What is DVA Community Nursing?

Community Nursing provides clinically required nursing and personal care in the comfort of your own home. Our services are designed to support eligible DVA clients in managing their health, maintaining independence and improving their overall wellbeing and quality of life.

Chris Barnard Health is a contracted DVA Community Nursing provider. If you’re eligible, DVA funds the service, so there’s no cost to you.

What Community Nursing can include

  • Comprehensive nursing assessment
  • Clinical nursing
  • Wound care
  • Medication management
  • Continence management
  • Chronic disease support
  • Clinical monitoring
  • Post-hospital nursing
  • Personal hygiene, showering and dressing where clinically required
  • Palliative care where appropriate
  • Mental health nursing and support
  • Ongoing assessment and care planning
  • Coordination with your GP and other health professionals

Who is eligible?

Eligibility is based on DVA entitlement and assessed clinical need, not age. Eligible clients may include

Gold Card holders

Veterans with a Veteran Card – All Conditions (Gold Card).

Eligible White Card holders

Veteran Card – Specific Conditions holders, where care relates to an accepted service-related condition or other DVA-funded treatment eligibility, such as Non-Liability Health Care (NLHC) or Provisional Access to Medical Treatment (PAMT). We confirm White Card eligibility with DVA before services begin.

Eligible war widows and widowers

Including war widows and widowers with a Gold Card.

Not sure which card they have, or whether they’re eligible?

Contact us anyway. We’ll help you work it out.

Helping a veteran or war widow/widower in your family?

You can contact us too. Many of our enquiries come from partners, sons, daughters and carers trying to work out what Mum, Dad or their partner is entitled to.

You don’t need to understand the DVA process. We can help you work out what’s needed and guide you through accessing the service.

Not sure where to start? Talk to us and we’ll help you understand the process, from referral through to care at home.

How it works

Talk to us about DVA Community Nursing

Fill in what you know. You don’t need to know which card they have, whether they’re eligible, or what referral is needed.

Refer a Client

GPs, hospital doctors and discharge teams, and Nurse Practitioners can refer eligible DVA clients directly to CBH.

A written referral can be made using the DVA Community Nursing Referral Form (D9389), your own letterhead, or a provider referral form with the required information.

Supporting veterans? Let’s work together.

RSLs, ex-service organisations, veteran advocates, veteran hubs, DVA rehabilitation providers and aged care providers can connect veterans and families with us. We’ll take it from there.
